Recent upgrades to My Health Record
(MHR) include a new consolidated view of immunisation details from the
Australian Immunisation Register (AIR) and the individual’s record, making it
easier for healthcare providers to view their patients’ information.
To support the Australian Government’s COVID-19 vaccination rollout, data from
all states and territories is now flowing into MHR. Healthcare providers can
access this data through their clinician information system or National
Provider Portal.
The vaccination data will appear on the MHR as soon as it is reported to the
AIR. Mandatory reporting legislated in March
means registered vaccination providers are required to report the
administration of COVID-19 and influenza vaccinations to the AIR.

Another clear fail because of the idiocy of having a secondary system (the #myHealthRecord) hold a copy of data that exists in a primary trusted form elsewhere – in the AIR. You only keep 2 copies of any data in two different places if you have no choice – synchronisation issues are inevitable.
If you want to display the data surely you just look it up in the primary system? No errors and no possibility of conflicting data. If you can’t find the data say so and suggest the citizen refer to the AIM for clarity.
Work around on work around yet again!

The original PCEHR was supposed to do just that for the exact reasons you give - multiple copies of the same data in different places is dangerous (especially with health data).
NEHTA couldn't deliver this functionality at any level so the PCEHR and now the myhr is just a dump of a database that needs constant feeding and with no data integrity.
Most of what is in myhr comes from and goes to somewhere else. It certainly does not reduce data fragmentation.
There are two very critical shortcomings of myhr.
1. You do not know if what you see is all you could see. Given that it cannot hold history, this it is guarantied not to be complete.
2. You do not know if what you see is the latest.

I guess this happened when we shifted from spitting paint on a wall to using clay tablets- some just cannot let go of the past. Sad really, they should step aside and let evolution continue.
Why would they step aside? They are doing a job that cannot be done, but which their bosses believe can be done. It's a job for life. What more could you want? You are protected from failure, success and automation. You won't find yourself replaced by a computer, now would you?
10:31 AM, had not thought to look at it that way. You raise an interesting point. After years of investing heavily and telling people they need to change, even going as far as to persecute the fax standard. They themselves are unwilling to change or even consider better alternatives.
Fools with primitive tools are still fools.
