A reminder just how vulnerable people can be exploited using these big systems that claim to have defends in depth, access controls and monitoring. As they go BAU things can get sloppy or neglected.

Wow that is concerning. How much goes unnoticed and unreported? We will never know. Clearly systems fail and fall into complacency. Far to much emphasis these days in on cultural rubbish and not on the duties. Took a longtime for this report to surface and has gone largely unreported, almost as if this is all far to common to be news worthy.
Anon 7:34 AM. Likely not that is is not news worthy, more likely it is the volume of mishaps and problems our trouble governments and bureaucrats create each and every day. Just impossible to fit it all in. I do agree - they need to pull their heads in, stomp pampering their feelings and do some bloody work for a change.
